Medicare Costs in Retirement: How Much Should You Budget?
A realistic Medicare retirement budget should include much more than the Medicare Part B premium.
Medicare isn’t free. Your total cost depends on which parts of Medicare you use, your income, your healthcare usage and any additional coverage you purchase.
In 2026, the standard Medicare Part B premium is $202.90 per month, and the Part B deductible is $283 per year.
Most beneficiaries don’t pay a monthly Part A premium, but Part A has deductibles and coinsurance. Prescription drug and supplemental-plan costs vary.
Medicare Part A
Most people pay $0 monthly premium for Part A because they or their spouse have enough Medicare-covered work history.
If you must buy Part A:
30–39 quarters of coverage: $311 per month
Fewer than 30 quarters: $565 per month
$1,736 per benefit period
This isn’t an annual deductible. More than one Part A deductible can potentially occur in a year because it applies by benefit period.
Hospital Coinsurance
Days 1–60: $0 after applicable deductible
Days 61–90: $434 per day
Lifetime reserve days: $868 per day
Skilled Nursing Facility Coinsurance
For qualifying Medicare-covered skilled nursing facility care:
Days 21–100: $217 per day in 2026
Standard premium:
$202.90 per month
Higher-income beneficiaries may pay more because of IRMAA.
$283 per year
After the deductible, Original Medicare commonly pays 80% of the Medicare-approved amount for covered Part B services and you’re generally responsible for 20%, subject to Medicare rules.
Part D costs vary by plan.
The maximum standard deductible permitted for a Medicare drug plan in 2026 is:
$615
Some plans have a lower deductible or no deductible.
For covered Part D drugs, the 2026 annual out-of-pocket threshold is:
$2,100
Once qualifying out-of-pocket spending reaches that amount, catastrophic coverage applies under the Part D benefit.
If you choose Original Medicare, you may purchase a Medicare Supplement policy.
You pay a separate premium for the Medigap policy in exchange for help covering some of Original Medicare’s deductibles and coinsurance.
Premiums vary by:
Medicare Advantage plan premiums and cost sharing vary by plan.
Some plans have a $0 additional monthly plan premium, but you generally continue paying your Medicare Part B premium and can still have copayments, coinsurance and other expenses.
Higher-income beneficiaries can pay additional amounts for:
Medicare Part B
and
Medicare Part D
through the Income-Related Monthly Adjustment Amount, or IRMAA.
In 2026, IRMAA begins above modified adjusted gross income of:
$109,000 for individual filers
or
$218,000 for married couples filing jointly.
How Much Does Medicare Really Cost in 2026?
When someone asks me, “How much does Medicare cost?” I don’t give them one number.
There are really several layers:
Medicare Part B premium
Potential IRMAA
Drug coverage
Medigap or Medicare Advantage
Your actual healthcare usage
The biggest planning mistake is looking only at the monthly premium.
What matters is your total potential annual healthcare cost.
